Security Access Application

Access within the Division of Chemistry and Biological Chemistry is restricted by security card-readers which prevents unauthorized persons from entering the laboratories. Students or staff members who are working in the laboratories have to seek for permission from the respective laboratory Principle Investigator or Laboratory Manager before entering the laboratory.

Students or staff members are required to complete 3 tasks before access can be granted.

STEP 1

Completion of risk assessment via Workplace Risk Assessment System (WRAS)

Please go through the risk assessment template and WRAS User Guide before conducting your risk assessment via the WRAS.

Upon completion of the 3 tasks, both the Security Access Application form and Safety Acknowledgement form have to be completed, signed and attached together before submitting to SPMS Chairs' Office level 4 reception counter for processing. Incomplete forms will result in delays in processing.

Please note that processing time for security access application may take up to 7 working days.

 

Damaged & Faulty Cards

For damaged or faulty staff cards and matriculation cards, please approach the relevant offices for replacement cards.

Students may apply for replacement matriculation cards via StudentLink -> Academic Matters -> Matriculation Card Replacement.

Staff may report personally at the Office of Human Resources for a replacement staff card
